IUI vs IVF: Which Fertility Treatment Is Right for You?

IUI vs IVF: Which Fertility Treatment Is Right for You?

FertilityBy Stepcare Team16 October 2026

If a fertility specialist has mentioned both IUI and IVF, the difference matters enormously for your time, your money, and your chances. They are not two versions of the same thing.

The short answer: IUI places washed sperm directly into the uterus and relies on natural fertilisation inside your body. IVF retrieves eggs, fertilises them in a laboratory, and transfers a resulting embryo back. IUI is simpler, far cheaper, and has lower success per cycle. IVF is more involved, considerably more expensive, and has substantially higher success per cycle.

How Each Procedure Works

IUI (Intrauterine Insemination)

Your cycle is monitored by ultrasound, sometimes with mild ovulation-inducing medication. When the follicle reaches maturity, a trigger injection times ovulation precisely. A semen sample is processed in the laboratory to concentrate the most motile sperm, then placed directly into the uterine cavity through a thin catheter. The procedure itself takes a few minutes and feels similar to a Pap smear.

Fertilisation then happens naturally inside the fallopian tube.

IVF (In Vitro Fertilisation)

Your ovaries are stimulated with injectable hormones for around 10 to 12 days to grow multiple follicles, monitored by regular scans and blood tests. A trigger injection matures the eggs. Eggs are retrieved under sedation through a transvaginal needle. In the laboratory, eggs and sperm are combined, either conventionally or by ICSI where a single sperm is injected into each egg. Resulting embryos are cultured for 3 to 5 days. One embryo is transferred into the uterus, and any surplus good-quality embryos can be frozen.

When IUI Is the Right Choice

IUI is appropriate when the pathway from sperm to egg is intact and the problem is one of access or timing. Specifically:

  • Unexplained infertility where all tests are normal
  • Mild to moderate male factor with adequate total motile sperm count after washing, generally above 5 to 10 million
  • Cervical factor, where cervical mucus impedes sperm passage
  • Ovulatory dysfunction being treated with ovulation induction
  • Sexual dysfunction or inability to have timed intercourse
  • Use of donor sperm
  • At least one patent fallopian tube, confirmed on HSG or HyCoSy

The important condition is tubal patency. If both tubes are blocked, IUI cannot work, because fertilisation depends on the egg and sperm meeting inside the tube.

When IVF Is the Right Choice

IVF bypasses the tubes entirely and controls fertilisation in the laboratory. It is indicated when:

  • Both fallopian tubes are blocked or significantly damaged
  • Severe male factor, where ICSI is needed to achieve fertilisation
  • Advanced maternal age, typically 38 and above, where time is the limiting factor
  • Diminished ovarian reserve
  • Moderate to severe endometriosis with distorted pelvic anatomy
  • Three to six failed IUI cycles
  • Recurrent pregnancy loss where embryo testing is being considered
  • Genetic conditions requiring preimplantation testing

How Many IUI Cycles Before Moving to IVF?

Most fertility specialists recommend three IUI cycles, and up to six in favourable cases, before escalating.

The reason is that IUI success is roughly constant per cycle rather than improving with repetition. If three well-monitored cycles with adequate sperm parameters have not worked, a fourth is unlikely to differ meaningfully, and continuing consumes months that matter more as age advances.

Age changes this calculation sharply. A 29-year-old can reasonably attempt four to six IUI cycles. A 38-year-old is often advised to attempt one or two, or to proceed directly to IVF, because the decline in egg quality during those additional months has a larger effect than the incremental IUI chance.

Success Rates: What the Numbers Actually Mean

Success Per Cycle

IUI, under 35

10 to 15 percent

Higher with medicated cycles

IUI, 35 to 40

8 to 10 percent

Declines with maternal age

IUI, over 40

Under 5 percent

Often bypassed in favour of IVF

IVF, under 35

30 to 40 percent live birth

Varies by clinic and protocol

IVF, 35 to 37

25 to 30 percent

IVF, 38 to 40

15 to 20 percent

IVF, over 40

Under 10 percent with own eggs

Donor eggs change this substantially

Always ask a clinic for live birth rate per cycle started, not pregnancy rate per embryo transfer. The two numbers can differ dramatically, because cycles cancelled before transfer are excluded from the second figure. Live birth per cycle started is the honest metric.

The Cost Calculation Over Multiple Attempts

Comparing a single IUI cycle at Rs. (Call 9606910113 for cost) against a single IVF cycle at Rs. (Call 9606910113 for cost) makes IUI look obviously cheaper. The comparison changes when you account for success rates.

Three IUI cycles at Rs. (Call 9606910113 for cost) each is (Call 9606910113 for cost) with a cumulative chance of roughly 30 to 35 percent for a woman under 35. One IVF cycle at (Call 9606910113 for cost) offers 30 to 40 percent in the same group, plus frozen embryos for future attempts without a full repeat cycle.

For younger women with favourable parameters, starting with IUI remains sensible. For women over 37, or where male factor is severe, going straight to IVF frequently costs less in total and takes less time.
